Rehabilitation Center

Five Oaks Rehabilitation team consists of in-house licensed therapists in the fields of physical, occupational and speech-language therapy. We have recently added a new gym on our short term rehabilitation unit, furnished with new, state-of-the-art equipment. An ADL kitchen suite offers our residents the ability to practice preparing meals and washing and drying laundry in preparation of returning home. Our Rehabilitation Department also offers Home Assessments to residents at discharge to determine any recommendations for a safe transition to home.

Physical Therapy

Physical Therapy (PT) trains residents in the areas of ambulation, balance, transfers and lower extremity strengthening. Modalities to assist in neuromuscular retraining and pain management are also available.

Occupational Therapy

Occupational Therapy (OT) focuses on assisting residents in returning to their maximum functional level of activities of daily living. OT also assists in positioning and contracture management with splinting.

Speech-Language Pathology

Speech-Language Pathology (ST) offers services for residents with cognitive, memory, language, voice, fluency and speech deficits, as wells as problems swallowing. ST also assists with augmentative/alternative communication for those residents who require an alternative means of communication, other than traditional speech.

Our Prosthetic Prowess Rehabilitation Program

 The Rehab Department at Five Oaks Rehab has developed a program to help amputees. Our Physical and Occupational Therapists combine with Prosthetists to help patients from the start of their rehab program all the way through a customized prosthetic fitting. 

A Prosthetic is an artificial device that replaces a missing part of the body (e.g., artificial leg) and “Prowess” is having expertise in a particular field and also means courage or bravery. 

Each day more than 500 Americans lose a limb. Becoming an amputee does not mean that life as you know it has ended, it simply means you need to learn to do things differently. The multidisciplinary team at Five Oaks Rehab is ready to help!
